Before Your Visit
In our previous article, Step One: Find Your Healthcare Provider, we discussed important factors to keep in mind when selecting a provider for your healthcare needs. This week, we’ll cover how to prepare for your visit.
We want you to get the most out of every visit, and as your Patient Centered Medical Home, we are committed to setting healthcare goals with you to provide you with the best care possible. We recommend you always come to your visit prepared with your questions and concerns. Here are a few things to keep in mind.
What kind of visit are you coming in for? Is it a general check-up, a follow-up, or a new concern? If you are considering big life choices like getting pregnant or radical changes in diet or lifestyle, let your Clinician know so they can work with you to update your healthcare plan. It’s a good idea to take notes on any symptoms you may be having, including details on timing, duration, and level of discomfort. If you take regular medication or supplements, make sure you make a list or bring them in, so your Clinician has a complete understanding of your healthcare needs.
We recommend making a short list that includes the concerns or questions you wish to address in order of priority. It’s common for patients to have something slip their mind during or after their Clinician visit, which is why our Patient Portal has an area for you to send messages directly to your Clinician. You can visit the “In Patient Portal” section of our Getting Started page for a step-by-step guide on how to check and send messages.
If you are a new patient with us or haven’t updated your patient information recently, remember to prepare any paperwork or insurance information you may need prior to your visit. Filling out your patient health history and questionnaires through our Patient Portal helps us check you in for your appointment more efficiently and helps to reduce wait times.
Need to cancel or reschedule your visit? We understand that schedule conflicts happen and request that patients provide at least 24-hour notice. You can read more about our appointment and cancellation policy here.
